Role

The Support Worker works within a team providing services to HWH clients. The role supports clients to remain independent, assist in social inclusion and maintain a healthy home environment. 

Responsibilities 

Providing domestic assistance, personal care, respite, and social and emotional support to people in their homes. This includes but is not limited to;
  • Assisting clients with their personal care needs – this may include toileting, bathing, showering, oral hygiene, and other personal hygiene needs. 
  • Assisting clients with cooking, food preparation and meal planning. 
  • Performing light domestic duties such as (but not limited to) vacuuming, laundry, general cleaning, dusting, washing dishes, spot cleaning windows, spot cleaning carpets, making beds, and essential home maintenance. 
  • Transport to medical appointments, social engagements, shopping (including assisting with shopping) and other outings as required. 
  • Supporting carers of clients and assisting them to maintain their caring relationships. 
  • Delivery of services that promote the dignity, independence and empowerment of clients. 
  • Adhering to the documented and approved Care Instructions. 
  • Monitoring any changes in client needs and reporting them to the Client Care team. 
  • Immediately report any changes in client conditions or incidents to the Client Care team.
  • Maintaining up-to-date, accurate client shift notes in line with HWH requirements. 
  • Report all emerging risks, issues and incidents to their manager or appropriate officer.

Background and Experience Requirements 
Essential 
  • Current First Aid
  • NDIS Worker Check
  • Willingness to undertake and the ability to pass competencies training and other mandatory training as required
  • Details of 2 previous or current employers for the employment reference
  • A phone capable of supporting the current business apps
Desirable 
  • Current CPR Certificate
  • Previous experience in a similar role
  • Certificate III in Aged Care or Certificate III in Individual Support or equivalent 
  • Certificate IV in Disability
  • Current Driver's licence
  • Motor Vehicle including registration and comprehensive insurance policy
Where applicable
  • Working with Children Check 
  • Willingness to undertake a pre-employment medical and/or functional assessment (including drug and alcohol screening)
  • Visa holders to provide information to enable a VEVO check.
  • Details of secondary employment as required by the NDIS.
  • Advise HWH in writing of any conditions that may impact the ability to carry out the responsibilities required of the role.
