How we manage care competency

HWH manages clients in their homes and occasionally in a hospital or related facility. Many of these clients are living with age-related chronic diseases and disabilities.

Care for clients such as these requires Support Workers with higher skill levels, and in many cases, there is a need to ensure the skills are current. Currency in this context implies at least an annual review of the related materials and an assessment by a Clinical Care Manager.

Staff Self Assessments
HWH performs staff reviews on an annual basis which consist of a 
  • Support Worker self-assessment followed by a 
  • Manager review. 
This self-assessment allows the staff to assess/critique their competency and allows HWH if an issue becomes apparent before the expiration of their course, to provide additional training.

Client Directed Training
As clients are Onboarded, a Training Group is created with courses assigned that cover their specific care needs. 

When Support Workers are assigned to the client, they are added to the client’s Care Team and automatically enrolled in the necessary training courses. 

The HWH Learning Management System, TalentLMS, notifies them to ensure they have the necessary training and can deliver the required care competently. 

For example, the team associated with client Smith was established when he was onboarded. The medical conditions are noted in the client’s Care Plan.

Once workers are assigned to Team–Smith, they are automatically enrolled in the above courses, and the Training and Clinical Care Manager can track their progress. 

Courses that require annual refreshers can also be assigned to ensure the continuing education and currency of best practices. 

As Higher-Intensity tasks and conditions are identified related to HWH clients, the courses are updated accordingly. Following are some of the support courses.
Certification
Talent(LMS) allows HWH to put specific timeframes reflecting the currency of courses. At the expiration of the given time or at some time before, Talent notifies the Support Worker of the need for them to re-sit the course and pass the related competency quiz.

Course Certificate expiration dates range from 1 month to 3 years, and the Support Worker is notified to retake the course from the date of the certificate expiration to 30 days before.

Onsite Review
Aside from the Annual competency check to go with the Support Worker self-assessment and the assignment of a Support Worker to a client with a condition that requires a new competency, they are under constant review by the HWH Clinical Care Managers.
